It is paid in connection with the organization of the burial of an insured person, pensioner, annuity holder or a family member of these people. Who is eligible?
Funeral allowance may be received by:
1. Natural persons – e.g. family members, but also other persons who actually covered the funeral costs.
2. Institutions – e.g. funeral homes, social welfare homes, commune, parish or other entities that paid for the funeral.
Benefit amount
• The maximum amount of the benefit is PLN 4,000 – regardless of the actual costs incurred.
• If the funeral costs are borne by more than one person/institution, the benefit amount may be divided proportionally.
Conditions of granting
A funeral allowance is payable if the deceased:
• Was covered by social insurance (e.g. employed, running a business).
• He was a pensioner or an annuity holder.
• Was a family member of the insured person/pensioner/annuitant (e.g. spouse, child, parent).
1. Documents required by ZUS :
• Completed application for funeral allowance (form Z-12).
• Death certificate of the deceased person (original or copy).
• Documents confirming coverage of funeral costs (e.g. invoices, bills).
• A document confirming relationship with the deceased person (if the application is submitted by a family member).
2. Where to submit?
• The application can be submitted in person at a ZUS branch, sent by post or via the PUE ZUS platform.
3. Deadline :
• The application must be submitted within 12 months of the date of death .
ZUS considers the application and pays the benefit within 30 days of submitting a complete set of documents.
